Continuum of Care Facilities

Continuum of Care Facilities are communities with long-term agreements guaranteeing lifelong shelter and access to all stages of health care services. A resident of a Continuum of Care Facility can typically enter in any stage, knowing they can add services as needed. Services offered may include meals, recreation, transportation, home maintenance, grooming or personal care assistance, and medical assistance. There are a variety of payment styles, and should be discussed in detail with the providers.
